Either multiverse or simply they exist in the Whoniverse and watch the in-universe Doctor Who series. I prefer the latter.
I was joking.Earth-1218 is supposed to literally be our reality, so likely simply a similar world, like Gwenpool's universe, Stan Lee's universe (from Spider-Man (1994)), the Deadpool Kills the Marvel Universe real world, etc.
Those characters who appeared don't exist in real life. If they're based on real people, they didn't literally meet the Doctor.
